Membership Plans


Given our focus on preventative dentistry, we offer 3 different membership plans for patients, considering individual needs and preferences. Please contact us if you require further information.

  • Two hygiene appointments per year including scale and polish and periodontal advice
  • Two dental health examinations per year
  • Up to two x-rays per year
  • Routine oral cancer screening
  • 10% discount on treatment
  • Up to one emergency assessment per year (excluding treatment charges)
  • Diet and oral hygiene advice
  • Membership card with 24 hours helpline numbers for dental emergencies at home or abroad
  • Eligibility to request assistance from the Worldwide Dental Emergency Assistance Scheme

Joining is very simple. All you have to do is complete a registration form for us and a Direct Debit mandate and authorisation form for DPAS. In addition to your first monthly payment, an initial registration fee of £10 per person will be payable by you to DPAS and will be included in your first Direct Debit payment. If you choose to leave the plan for any reason you can do so by simply giving us one month’s notice.

Crouch Street Dental Care Plan


Our plan is available from £15.60 per month and covers:

  • All regular dental health examinations
  • All routine health appointments including scale and polish and periodontal advice as clinically prescribed
  • All small routine x-rays
  • All routine fillings as clinically prescribed
  • All routine crown and bridge work (laboratory fees excluded)
  • All routine examinations
  • Root canal treatment
  • Extensive preventative advice including diet and oral hygiene advice
  • Membership card with 24-hour helpline numbers for dental emergencies at home and abroad
  • Eligibility to request assistance from the Worldwide Dental Emergency Assistance Scheme

The monthly plan cost includes the charges for management and administration payable by you to DPAS.

Crouch Street Children’s Plan


6-11-year-olds for $5/months 12-18-year-olds for £10/month Both plans cover:

  • Routine dental health examinations
  • Routine hygiene appointments including scale and polish and periodontal advice as clinically prescribed
  • Routine small x-rays
  • Routine dental treatment as clinically prescribed
  • Prevention advice and treatments including fissure sealants
  • Choice of appointment times outside school hours
  • Diet and oral hygiene advice
  • Membership card with 24-hour helpline numbers for dental emergencies at home or abroad
  • Eligibility to request assistance from the Worldwide Dental Emergency Assistance Scheme

The monthly plan cost includes the charges for management and administration payable by you to DPAS. Treatment not covered by this plan can be paid separately.

Children 0-5 years will be seen free of charge but won’t be eligible to request assistance from the Worldwide Dental Emergency Assistance Scheme.


There is no need for an assessment. All you have to do is complete a registration form for us and a direct debit mandate and authorisation form for DPAS, on behalf of your child.

In addition to your first monthly payment, an initial registration fee of £10 per child will be payable by you to DPAS and will be included in your first Direct Debit payment.

If you choose to cancel your child’s plan for any reason you can do so by simply giving us one month’s notice.

Worldwide Dental Emergency Assistance Scheme


Whilst a member of the dental plan, should you suffer a dental trauma, a dental emergency or be diagnosed with oral cancer you will eb eligible to make a request for assistance from the Worldwide Dental Emergency Assistance Scheme. The Scheme responds to

If your request for assistance is accepted by the Scheme Manager, you will be eligible to receive benefit payments from the Scheme in the following situations (up to the limits set by the Scheme and published from time to time):

  • The cost of dental treatment by any dentist up to a maximum of £10,000 for any one incident of dental trauma
  • The cost of:
    • Emergency callouts
    • Pain relief or emergency temporary treatment
  • A specified amount for each complete 24-hour period of hospitalisation wholly or partly under the care of a consultant who specialises in dental or maxillofacial surgery
  • A specified amount if you are diagnosed with oral cancer and this is the primary cancer site

You will also have 24-hour access to a worldwide dental emergency helpline.
